Notes:

• This form must be used to register a team.
• Remember only the first 25 schools to completely register will receive a spot in the contest, and a second team may register on a space available basis.
• Please submit a separate form for each team you wish to register. Identify the different teams by number, as #1 and #2.
• The team sponsor must be registered for the CCSC:Southeastern Conference. (Students may also register as a conference attendee at a reduced rate, but it is not required.)
• Your registration will be complete upon receipt of this form and receipt of the registration fee and receipt of the sponsor's conference registration. The fee is $20 per team, plus $10 per individual team member. (More details and the form for submitting fee payment can be found here.)
• Please send an electronic copy of one contest problem that could be used in the problem set to Andy Digh. Team sponsors should not discuss this problem with their team. If your problem is selected, additional test data will be requested and you will be invited to help judge.
• Before submitting this form, please be sure to read all additional contest information thoroughly.
• Receipt of your electronic registration and problem submission will be confirmed by return e-mail. If you do not receive this confirmation, please contact Andy Digh to make sure that you are registered.
